Husband seeks info about wife's death

Srinagar, June 2: A man has sought information about the reasons of the death of his wife in Sher-e-Kashmir Institute of Medical Sciences (SKIMS) early this year.
As SKIMS authorities denied the information, the State Information Commission (SIC), directed the SKIMS authorities to provide the certified copies to the information seeker.
Details available with Early Times reveal that one Syed Mohammad Yaseen filed an application before PIO SKIMS Soura on 14.01.2016 for seeking information pertaining to death case of his wife Syed Kulsuma. PIO on 16.02.2016 provided him partly wrong information without covering letter. Thereafter, the appellant approached to First Appellate Authority (FAA) SKIMS on 17.02.2016. The FAA disposed of the First Appeal vide order dated 10.03.2016 without providing information/documents in the light of RTI application. The appellant accordingly requested the State Information Commission (SIC) for justice with direction to concerned respondents to provide him correct information as per his RTI application.
As per the records available, the appellant had filed RTI application before PIO SKIMS received in his office on 14.01.2016 for seeking information on five points pertaining to death case of Syeda Kulsuma (MRD No.00816606 dated 29.08.2014) . On 17.02.2016, the appellant filed First Appeal before Jt. Director SKIMS Soura who is also FAA. On scrutiny of the documents, some deficiencies were detected by the State Information commission (SIC), which were completed by the appellant on 23.03.2016.
In the instant case, the First Appeal has been decided by FAA/Jt. Director SKIMS vide order dated 10.03.2016 concluding that PIO has provided all information as requisitioned by the information seeker in vide his initial application.
"The HOD Radio-diagnosis will provide the name and full particulars of the Technician/Technician who was/were available in the x-ray room and entered the name of his patient in the X-ray register as desired by information seeker. Besides communication of DMRO and HOD Radio-diagnosis dated 25.02.2016 may also be supplied to information seeker" reads SIC's recent order.
PIO SKIMS filed counter statement/reply to the appeal on 29.04.2016 submitting that information forwarded by Medical Superintendent on 03.02.2016 was intimated to the information seeker vide letter dated 06.02.2016 providing information in respect of the RTI application. Thereafter, in compliance to the directions of the FAA, information about technicians, who were available in X-ray room were provided to the appellant on 24.03.2016. During proceedings in the Commission, the appellant admitted receipt of information but submitted that information dated 30.01.2016, 25.02.3016 and 23.03.2016 provided by HOD Radio-diagnosis SKIMS has not identified his name.
SIC order further reads: "PIO SKIMS is directed to communicate the name of HOD who has signed the information as per references given here in above. The appellant further submitted that some information provided to him by the PIO including copy of his own application provided by PIO has not been attested/certified. As per the definition of Right to Information, the citizen has a right to get certified copy of information. Therefore, PIO is directed to attest/certify all such information provided to the appellant, which has not been attested by him earlier"
While talking to Early Times RTI Activist Umar Javaid said that employees of the department of Radio Diagnosis SKIMS are callous and treat patients as cattle. Umar Javaid has also sought some details from SKIMS authorities which lead to death of his mother around a month back.
